Home Hospice of Fannin County is a 501(c)(3) non-profit organization founded in 1982 as the first and only non-profit hospice in our service area. We are a 5-STAR hospice Medicare provider and for ten years, we have received the HOSPICE HONORS award for family satisfaction while serving generations of families in our community. Since 2019, the community has voted us each year the Best Hospice Services in Texoma.
Our commitment is to the care and support of the chronic and terminally-ill patient and their family. We help them address physical, emotional, psychosocial, financial or spiritual symptoms, needs and issues of their disease process. We serve all in our community regardless of funding sources. This care and support is done through the services of an interdisciplinary team including physicians, nurses, social workers, aides, chaplains, volunteers and office staff.
The Pathways Palliative Supportive Care Program is for those with an advanced, chronic or serious illness who are struggling with pain and symptom management, support and resources. The focus is on the patient and their family to help them manage their disease symptoms so they may remain at home without costly hospitalizations. We work in coordination with their physicians allowing patients to still seek curative treatments.
Our Bereavement support is coordinated through our Grief and Loss Center. We offer weekly support groups, individual 1-on-1 counseling and our grief support for children 7-12-years old. The Camp Dragonfly, kids camp, is in its 16th year!
